The MolDx program uses a number of checklists and Excel documents to solicit and organize information for test review. Some of the documents are general applicability, others are tied to specific LCDs (e.g. pharamacogenetics). See also an important overview of FAQ for the tech assessment documents - here. And there is also a "MolDx Manual," though not updated since 2019 (16pp, here.)
